Pediatrics

The Department of Pediatrics is a major and oldest(34years) departments of D.Y. Patil medical College, Kolhapur. It offers comprehensive and holistic pediatric health care. The Department has 3 units with 30 beds each that is 90 general pediatric ward beds, a Pediatric ICU with 10 beds and NICU with 30 beds and Pediatric Emergency room.

The General Pediatric OPD is open on all working days of week. OPD provides growth assessments, Developmental assessment, vaccination, well baby and consultation for sick child.

The inpatient ward provides complete care to children suffering from acute and chronic diseases which require tertiary care services.

Apart from General pediatrics the Department of Pediatrics runs following sub specialty services for children from age 0 (newborns) to 18 years of age.

  1. Department of Neonatology – Post natal ward and state of Art Level 3 ,30 bedded NICU, High risk newborn clinic.
  2. Department of Pediatric Critical Care – State of Art 10 bedded PICU with pediatric ventilation, dialysis and intensive critical care monitoring facility.
  3. Department of Pediatric Hematology – Blood disorders and Thalassemia.
  4. Department of Pediatric Surgery – surgical care of newborns to adolescents.
  5. Pediatric Emergency Services – for all types of Pediatric Emergencies who need urgent attention, close observation and monitoring
  6. Adolescent Care Services – Teen Care Clinic with Adolescent specialist
  7. Infectious Diseases Clinic and Tuberculosis Clinic – Expertise in diagnosis and management of common and rare infections in children and complex pediatric TB cases (NTEP center)
  8. Pediatric respiratory and asthma clinic -subspecialty clinic run by Pediatric Pulmonologist for children with chronic respiratory diseases with spirometry.
  9. Pediatric Nephrology – Expertise in and management of Acute and chronic kidney disorders in children.
  10. Pediatric cardiology – In house pediatric cardiologist with Echo and color Doppler facilities.
  11. Pediatric Neurology – Special clinic for assessment and follow-up of children with Neurological disorders.

The Department of Pediatrics was initially established in 1989 in CPR hospital to which college was affiliated, Departmental Head was Dr. Ashok Kore (1989-2011). It was shifted to Dr. D.Y. Patil hospital and Research institute, kadamwadi in 2003. The DCH programme started in 2008 and the MD programme in 2005. The Head of the Department was Dr. Anil kurane (2011-2023). Dr Nivedita Patil is Head of the Department at present. The services have expanded to include Neonatology, Pediatric Intensive care, Pediatric Nephrology, Pediatric cardiology, Pediatric Rheumatology, Pediatric Neurology, Developmental Pediatrics, Pediatric Hematology, Pediatric Pulmonology, Adolescent care and Infectious diseases.

At present department has intake of 9 M.D pediatrics post-graduation seats / year. Fellowship programs in sub-specialties like Neonatology and pediatric intensive care are also being conducted by D.Y. Patil education society institution deemed to be University, Kolhapur.

Pediatric Nephrology clinic:

Primary objective:

  1. Registration of children suffering from Acute and Chronic kidney disorders. Detailed record is maintained in hard and soft copy as an excel sheet document.
  2. To recognize and diagnose specific kidney disorder and provide optimum care
  3. To address other co-existing co morbidities in children with kidney disease
  4. To educate and counsel care givers with help of information booklets and AV Aids
  5. To have Written treatment plan for every patient and facility for inter departmental discussion

Secondary objectives:

  1. To conduct and organize training modules and workshops in pediatric nephrology for undergraduate, Post graduate students and faculty.
  2. Data entry and storage

Pediatric Respiratory and Asthma clinic:

Primary objectives:

  1. Registration of all children with uncontrolled or poorly controlled asthma across all units in the asthma clinic. A detailed file is maintained for each patient in the form of hard copy as well as an excel document. Spirometry is conducted of children.
  2. Develop and implement treatment plans for children with respiratory issues
  3. To maintain long term, follow up both remotely by tele consults and in person at the clinic
  4. Offer education to parents and caregivers on managing and preventing respiratory issues to children including proper use of medications and recognizing early signs of exacerbation.
  5. To have a written asthma action plan (short term and long term) for every patient
  6. Provide emotional and practical support to children and their families regarding impact of respiratory conditions on overall wellbeing

Secondary objectives:

  1. Data Entry and storage
  2. Follow-up care to track the progress of child and make adjustments to treatment plan as needed
  3. Offering counselling and guidance to patients and their families addressing concerns, fears.
  4. Conduct research in asthma including publications, conference presentations and dissertations.
  5. Collaboration with educational institutions to create asthma or respiratory action plans for children, ensuring that necessary support is in place during school hours.
  6. Engage in community outreach programs to raise awareness about pediatric respiratory health, prevention strategies and available resources.
  7. Encouraging and facilitating the participation of pediatric patients in relevant clinical trials or research.
  8. To discuss difficult cases with other faculty in the department as well as experts in the field of asthma outside the institute as well.
  9. To report the activities of the clinic every quarter to the HOD and other faculty.

Pediatric Cardiology clinic:

Primary objectives:

  • Registration of all children who attend cardiology clinic, a detailed file is maintained of each patient in form of hard and soft copies or an excel document
  • Clinical examination is performed by pediatric cardiologist and chest Xray, ECG, ECHO, CT angiography as and when necessary.
  • To maintain long term follow of patient suffering from cardiac disease to educate and counsel caregivers regarding disease process with the help of AV AIDS, information booklets and cardiac models.
  • To emphasize on importance of compliance for treatment and follow up

Secondary objectives:

Well baby clinic:

Primary objectives:

  1. Regularly assessing and monitoring the physical and developmental health of infants and young children.
  2. Administering and tracking vaccinations to ensure children are protected against preventable diseases.
  3. Providing guidance and support for proper infant and child nutrition to promote healthy growth and development.
  4. Conducting developmental screenings to identify any potential delays or issues early on, allowing for timely interventions.
  5. Disseminating information about common health concerns, such as breastfeeding, sleep patterns, and common illnesses, to empower parents with knowledge.
  6. Educating parents and caregivers on essential childcare practices, including hygiene, safety, and early childhood development.

  3. Conducting Early Intervention Programs by collaborating with early intervention services for children with identified developmental delays, ensuring prompt and tailored interventions.

High risk new born follow up clinic:

Primary objectives:

  1. Follow up of NICU graduates which includes preterm<36 weeks, Low birth weight neurological problems, chronic lung disease, long ventilation days babies.
  2. Evaluation of growth (intergrowth 21) a development (GDM score) of high risk neonates.
  3. Detect early signs of Neuro motor delay and cerebral palsy.
  4. Consultation with speech therapist, ophthalmologist, physiotherapist, nutritional counselling and social support.

Secondary objectives:

  3. Conducting outreach programs to raise awareness within the community about high-risk newborns, their unique needs, and the importance of early intervention and support.

Hematology and Thalassemia clinic:

Primary objectives:

  1. Diagnosing and treating disorders of the blood and blood-forming tissues such as anemia, leukemia, and hemophilia.
  2. Assessing and managing blood clotting disorders, including thrombosis and bleeding disorders.
  3. Interpretation and analyzing blood tests, such as complete blood counts, coagulation profiles, and bone marrow biopsies, to aid in the diagnosis and monitoring of blood disorders.
  4. Providing genetic counseling and testing for inherited blood disorders, such as sickle cell disease or thalassemia.
  5. Establishing a systematic and timely blood transfusion schedule for patients with thalassemia to manage anemia effectively
  6. Monitoring and administering iron chelation therapy to prevent iron overload complications associated with regular blood transfusions.

Pediatric Neurology clinic:

Primary objectives:

  1. Identifying and addressing neurological disorders in children, ranging from common conditions like epilepsy and headaches to more complex issues such as neurogenetic disorders.
  2. Tracking and assessing the neurological development of children, identifying any delays or abnormalities that may require intervention.
  3. Specialize in the diagnosis, treatment, and management of seizures and epilepsy in pediatric patients.
  4. Addressing conditions affecting the muscles and nerves, such as muscular dystrophy and neuropathies, in order to optimize function and quality of life.
  5. Evaluation and managing neurological issues impacting a child’s cognitive and behavioral functions, including attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorders.
  6. Specialize in the diagnosis and treatment of headaches and migraines in children, considering both acute and preventive approaches.
  7. Addressing neurological issues that arise in newborns, including birth-related injuries and conditions affecting the developing brain.

Child guidance clinic:

ORT and Immunization clinic:

Primary objectives:

  1. Administering and promoting timely and age-appropriate vaccinations to protect children from preventable diseases, following national immunization schedules.
  2. Monitoring and tracking vaccine coverage rates within the community, aiming for high immunization coverage to establish herd immunity and prevent outbreaks.
  3. Educating parents and caregivers on the importance of oral rehydration therapy, including proper preparation of oral rehydration solutions and recognizing signs of dehydration.
  4. Monitoring the progress of children receiving oral rehydration therapy and provide appropriate follow-up care to assess their recovery and prevent relapses.
